Medium Productions Questionnaire

Kindly note, our minimum booking is 2 hours and all our bookings are prepaid. We also require a security deposit of $500 security deposit that will be held on the card used for this booking transaction. Once confirmed that the studio and building are left as found (no damages/missing items), we’ll refund the security deposit back. Depending on your bank it may take up to 7 business days to reflect on your statement.

I understand that all activities related to my booking must be contained within the studio.

I understand that all of my props/equipment must be stored inside the studio unit and I will not leave items outside the space.

I understand that I cannot bring big or heavy props/equipment (more than 3.5ft and/or 20lbs; light stands not included.) Otherwise, my booking will be adjusted to the appropriate production category

How many persons in total will be in the studio at a time? (Up to 15 maximum persons; the rate will be adjusted according to the appropriate production category when exceeded)

We have 1 guest parking space available that fits vehicles ranging from small cars to small cargo vans only. Please provide the plate number of the vehicle that will park in the building parking lot.
